Also treats children who have witnessed domestic violence in their home or suffered physical abuse or neglect. Treats non-offending parents and siblings, and any victim of crimes, also treats victims of human trafficking. Prenatal Plus

Description

Provide a home visiting program to at-risk pregnant women using a multi-disciplinary team approach. Each person is eligible to receive supportive services from a Care Coordinator/Nurse from Dept. of Health, a Behavioral Heal Practitioner from CFS and a Registered Dietitian from Nutritious Lifestyles. Each team member conducts an initial assessment and develops an integrated plan of care for ongoing services.
